As a 501C3 organization, we are exempt from sales tax. Use this form for purchases and/or vendors that charge sales tax.

If an employee is injured on the property, we have a process to make sure the employee and organization know what to do. This page has steps on what to do in the unfortunate event of an injury.

First Report of Injury Process  In the event of an injury, the operations team will walk you through the steps to get you the medical care you need. Please follow the steps below and if you have questions reach out to the Operations Team by emailing Joe@thepearl.church 

1. Seek Emergency Care  In the event of an emergency, the injured employee should seek immediate medical attention.
*Care is most important. Details and paperwork will come later. 

2. Report the Injury Right Away
A: No Treatment Needed/Wanted – Document, no need to report
B: Basic 1st Aid  – Document, no need to report
C: Further Care Needed  – Church Staff – Report to Ops – CEC Staff – Report to Admin 

3. Seek Medical Treatment  Seek medical treatment from designated medical provider.

4. Complete Notice of Injury Form
The Employee should report the injury in writing by the next business day.
Give Employee Notice of Injury Form.
Review and Complete with Them to ensure info is accurate  Not all boxes need to be completed.

5. File Claim with Insurance  In house workers comp rep will file claim with insurance company
1. Go to Brotherhood Mutual’s Website
2. File a Claim by Emailing form to the email listed 

6. Maintain Communication  Stay in Contact with the Operations Team 

7. Maintain Maximum Medical Improvement
Medical care continues until the injured employee reaches Maximum Medical Improvement (MMI), the point at which the injured has stabilied in recovery and no further reasonable or necessary treatment is expected to improve their condition.

When someone make a non-cash donation they may ask for a receipt. Use this template to make the receipt. (Please note, the donatee is responsibl for providing the dollar amount equivelent to the donation.)

Use this template for any paper signups. While we recommend using Planning Center signups for most things, paper signups can be useful for events once in and awhile. We recommend this one:)
